Each Client receives individualized Care, which is achieved by professional Care activity according to a bespoke Care Plan, as set out by an experienced Cygnet assessor, with the knowledge and support of the Client and their family. All details of Care to be given, medication records and all Care received are kept in the Client’s home.
Here at Cygnet Care, we believe that the support we administer is an integral part of total health Care and is planned and applied in co-operation with the direction of the General Practitioner, District Nursing team, the Client and their family.
We believe in the effective utilization of professional and non-professional personnel working together as members of the Health Team.
